Item Level Army

I have got 12 ilvl 650 followers, but for some or other reason have not got the achieve. some of these have been de-activated.. do you think this could be the reason for not getting the achieve? a couple of my followers have got higher level gear than weapon, could this also be the cause? e.g. lvl 660 gear and ilvl 645 weapon? (but all displaying ilvl 650 +)
would really appreciate some feedback...
(perhaps once I get all 20 up to 650 will be credited with both achieves?)

Wowhead comments seem to point to both of your suspicions, first it is bugged,second the followers must be active and you actually get the achievement on the upgrade after 10 active followers hit 650+
