Got this years ago from the quest, having to get poisoned by Mother Smolderweb and run back to Flame Crest with the debuff to get "milked" lol. Nasty indeed. The pet got a wicked audio if you click it.

Already stated on Worg Pup so I'll quote myself.

"Hoping someone else could check something. I just bought, to my surprise, both the Worg Pup and Smolderweb Hatchling from Karin <Stable Master> in Stormwind. I may not have looked properly but neither appeared to be of limited supply - but I bought both, learned them on my alt and checked the vendor again to find only a Rope Leash for sale. Anyone else encountered this?"

Edit: Appaears it was a kind of reimbursement of lost pets on an old toon!

En-Ay-Es-Tee-Why (or) N-A-S-T-Y = Nasty!!! think about it for a minute then you will see the light!! Looks like a few others got this before I posted or read other posts. I figured it out for myself back in 2004. The name of the quest bothered me and I just looked at it and got it in a few seconds. I used to use Petopia to find. The pet(s)- Spider /Worg pup was always BOP never able to be traded. I just found they both drop 4.3 luck of the dice if you group to get.

Got this guy today, along with the Worg pup, solo'd it on my lvl 74 hunter pretty easily, both dropped on the first run so from all reports the 100% drop rate looks true.

As of patch 4.0.6 this definitely drops from Mother Smolderweb in Lower Blackrock Spire. It seems to be 100% drop.

According to wowhead you can now obtain this again as of patch 4.0.6

This pet is now a rare that drops off Mother Smolderweb in LBRS. I had this drop on an alt that hadn't done the quest. Did not see it when I took in my main who does have this. Will provide a screen shot after I go again on another alt.

Here it is. I just did that a few minutes ago, Neither wolf dropped the puppy for me.

As per the sundering patch (4.0.3) the quest to obtain this pet is NO LONGER AVAILABLE.

Like it's been suggested, if you're aiming to finish the Worg Pup quest, might as well consolidate and finish the Smolderweb Hatchling quest.

And for a fun addendum, go for the Leeeeeeeeeeeeeroy! Jenkins achievement. It's in the same dungeon, just make a left turn instead of the right turn the moment you enter the instance. Be forewarned, the area-effect abilities of some classes post-patch 4.0 were tuned down.

Started collecting pets after you did these quests and discarded the pet boxes for lack of space (you heartless person, You)? Fear not! Just go to any stablemaster and talk to them, and they will give you back your pet reward!

Thanks to Warcraftpets.com I used the guide to obtain this pet and the Worg Pup. As a lvl 70 Druid I stealthed through the instance with virtually no problems.

I had thrown this pet away (thing I've never done to any other pet) because it was so ugly I never wanted to use it. If you did the same as me, remember you can get it back from npc in Dalaran Sewers!

Pssst! Come closer! Ok, I'm gonna tell you a secret. You know the name of the quest you used to have to complete to get this spider? "En-Ay-Es-Tee-Why". Well, it took me about a year before I realized what that meant. Yup… a whole year of wondering what this strange hyphenated word meant. Sometimes my obliviosity saddens me. *sniffle* What?
