Just caught one in Eastern Plaguelands.  It showed up as a secondary with an Infected Squirrel.

Found mine in Ruins of Kul'Zunda in Northern Stranglethorn as the main critter and found the rare version as the second to a Tree Python.

My first pet battle in Ashenvale, I found it second to a Forest Moth, well, I had caught the Moth, and you can't catch two pets in a single battle. After about an hour searching, I found one again second to a squirrel, and accidently killed it, I could have punched myself. Finally after about another forty minutes, I found it second to a toad, and caught it that time.

I caught one second to a rusty snail on the Zoram Strand, near Ruuzel's Isle (NW on the map)

This pet appears as the second to Maggots in Ashenvale, in and near Demon Fall Canyon.  It doesn't seem to show up for Maggots elsewhere in the zone, nor independently.
