Well the main thing with balance card trades is that the person with the bnet balance cards has to actually get the physical cards from brick and mortar stores like Best Buy and maybe Wal-Mart. For some reason Blizzard has not let us gift out bnet balance that you can buy from Blizzard.

So this usually complicates trades because not everyone can get the physical bnet cards which makes trading for them even harder.

Generally someone buys a balance card from, like Faelar said, a retail store like wal-mart or best buy. Then you trade ingame. All of the trades I've done, generally the person with the card will give the other person the code from the card in a whisper, works the same as if you gave them a tcg loot code. Only trade with people you trust, if you don't know them get a rapport from other trades. Generally in this community you build good references through other trades. When the code is validated then the other person will trade you the item you're after. These trades aren't always quick and can take a few mins so you just have to be patient.
